Statements
Mission Statement Christian Care Communities' mission is to enhance the journey of life for Older Adults.
Background Statement Christian Care Communities is Kentucky’s largest faith-inspired, non-profit provider of affordable senior living and long-term care for Older Adults. Founded in Louisville in 1884, today Christian Care serves over 5,000 Older Adults annually through an extensive network of senior communities, including: Independent Living; Assisted Living, Assisted Senior Care, and Memory Care; Skilled Nursing Care; Short-term Rehabilitation; Long Term Care; and Adult Day Care.
Impact Statement
- Hosted Leading Age and additional national conferences with community tours to promote Greenhouse Model and dementia care
- Became the first Life Plan community in Woodford County
-Consistently rated a top senior care provider for Kentucky 
Needs Statement
Current needs include:
- A 12-passenger van to transport residents to physician appointments, community events, and worship services
- A surrey for additional on-campus and around-town physical therapy, community event transportation, and therapeutic recreational travel between residents and their loved ones
- A volunteer recruitment and management program with software

Description

The Homeplace at Midway accommodates residents at all levels of care within four comfortable, homelike cottages. While each resident enjoys his or her own private bedroom and bath, other areas of the cottages, including the kitchen, living room, den and porch are shared.

The Homeplace campus features an Assisted Living cottage for residents who need periodic assistance with daily activities, such as bathing, dressing, eating, walking, etc. Our Memory Care cottage accommodates residents with Alzheimer’s disease, dementia, and other cognitive impairments. Two Skilled Nursing Care cottages serve residents in need of short-term rehabilitation or long-term care. Each cottage accommodates 12 residents, creating a close-knit community where residents can receive personalized attention and quality care.

Every level of care is delivered with the Best Friends Approach, ensuring each resident at The Homeplace is cared for at a deep, personal level.
